How to access Polish health service
EU citizens
-    If you are insured in your country of origin, and are visiting Poland, you will be treated by the Polish health service, based on the European Health Insurance Card (EKUZ), which should have been issued to you in your country of origin, before you traveled.
-    The EU students can also use the Polish health service based on the EKUZ Card.
-    Anyone who is insured in his country of origin and intends to stay in Poland for longer, needs to confirm their insurance details in the regional office of the National Health Fund (Narodowy Fundusz Zdrowia) by submitting an appropriate form; it might be the E-109, E-106 or E121.
-    E-106 is a form that entitles you to health service both in the EU country where your insurance contributions are being paid and in Poland
-    E – 109 is issued to the family members of a person who pays her insurance contributions in an EU country, and entitles her to medical treatment only in Poland
-    E-121 applies to pensioners who intend to move to Poland. Their insurance contributions are paid in EU and then transferred to Poland.


Non-EU Citizens
-    If you are a student and you wish to be insured in Poland you need to sign a health insurance contract in one of the NFZ offices. Your monthly contribution will be approximately 38 PLN. The fee varies, dependent on the type of services on offer, and at the moment it is 37.80 PLN. If you need to see a doctor you will have to show him your NFZ contract and a confirmation of payment
-    The Non-EU tourists have to be insured privately
-    Anyone who intends to stay in Poland for longer periods of time can sign a health insurance contract with the NFZ.
-    Anyone legally employed in Poland will be insured by their employer.
